Friday, May 06, 2016

The U.S. Department of Health and Human Services announced on Thursday that Reliant Medical Group is on of 18 healthcare organizations nationally to be named a Million Hearts 2015 Hypertension Control Champion. They are the only one in Central Massachusetts to be named.

“I applaud our team of doctors and all of our caregivers for earning this prestigious recognition, as it acknowledges their collaborative commitment to the good health and wellbeing of our patients. Staying closely connected to our patients – especially those with chronic conditions such as hypertension – is a critical element of successful treatment," said Tarek Elsawy, M.D., Reliant president and CEO.

Million Hearts 2015 Hypertension Control Champion

The Centers for Disease Control and Prevention (CDC) one of the major operating divisions of the Department of Health and Human Services, launched the Million Hearts Hypertension Control Challenge to recognize doctors and health  practices that achieve blood pressure control for at least 70 percent of adult patients and hypertension, also known as high blood pressure.

According to the CDC, high blood pressure is a major risk factor for heart disease and stroke, which are top five leading causes of death in the United States.

Approximately one in three Americans suffer from high blood pressure, a condition that contributes to almost 1,000 deaths per day in the U.S.

“At Reliant, we have developed a system of care anchored by extensive knowledge of our patients,” said Michael Sheehy, MD, Reliant chief of population health & analytics. “For instance, our robust electronic health record enables us to stay connected to our patients throughout our organization on an ongoing basis, thus aiding us in monitoring patients with conditions such as high blood pressure,”  said Michael Sheehy, MD, Reliant chief of population health & analytics.

Million Hearts is a national initiative focused on preventing one million heart attacks and strokes by the year of 2017, hypertension control is a critical step in realizing the goal.
