Worcester’s Autumn Village Earns Deficiency-Free Survey Rating

This survey is the Public Health department’s rigorous annual examination and is one of the top indicators of excellence for skilled nursing facilities. Skilled nursing facilities in Massachusetts and across the country are thoroughly surveyed annually and rated based on the following criteria:
- quality care
- safety
- administration
- food service
- nursing care and patient rights
“Each one of us is responsible for executing our duties and responsibilities to the best of our abilities for each resident we come in contact with, every day," said Jerry Shaffer, Administrator of Autumn Village. "DPH examines how successful we are in meeting our department-specific standards of care. There is nothing singular in the way in which excellence in patient care is performed. Every element of a patient’s care is measured as part of the total outcome. Every day we look at new ways to improve the care of our residents.”
The survey team
The survey team arrives unannounced at each facility once within a nine to fifteen month window. The team is comprised of at least one registered nurse and a social worker. They thoroughly review clinical records, tour the facility, and randomly interview patients, residents, family members, and staff members.=
Five-star rating
This deficiency free survey comes right after the facility earned a Five Star rating from the Centers for Medicare & Medicaid Services. Autumn Village is also Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Certified as well as a contracted Veterans Administration Skilled Nursing facility.
