Tuesday, Fallon Community Health Plan, (FCHP) announced an innovative new wellness program, the Healthy Health Plan, that provides $200 in cash to subscribers who are or become healthy. It’s the first plan in Massachusetts that includes such a cash reward as a standard benefit.

Through interactive tools and calculators as well as health coaching, the Healthy Health Plan will educate and empower our members to live a healthy lifestyle. It will also encourage preventive care, which makes health care more affordable for all of its members. The new plan will feature:

Health Assessment

The assessment is a questionaire that members can fill out to learn more about their individual health and deals with issues concerning weight, smoking, physical activity, healthy eating, stress, productivity and absenteeism, cancer screenings, flu vaccine, risky behaviors and symptoms of depression.

Risk Advisor and Wellness Workshops

After completing the Health Assessment, the Risk Advisor tool suggests appropriate Wellness Workshops for the insured based on any risks that he/she reports. Some Wellness Workshop topics include: how to make healthy lifestyle changes, disease prevention, and more.

Interactive tools and calculators

The Healthy Health Plan includes tools such as meal plans, exercise tutorials, cardio/strength training logs and food logs.

Health Coaching

Insured members can access personalized health coaching provided by nurses and other health care professionals. Coaching can be either web-based or by phone.

Member forums

In forums members can ask questions, share tips, while supporting and interacting with each other in their health and wellness goals.
Incentives

Subscribers will be eligible to receive up to $200 in financial incentives when they complete a Health Assessment and/or a customized health and wellness plan.
