Medicare Ranks Worcester’s Century Homecare One of Best in MA

This is wonderful news, not only for Century Homecare, but also for our patients and their families, said William von Rolappe, Director of Quality Assurances and Compliance for Century Homecare. "Reducing re-hospitalization is our top priority and the center of our care planning process. Such a low number of hospitalizations is particuarly impressive for Century because we serve a large population of mental health patients. These patients are at the highest risk for readmission partly due to complications with medicationas and social and econcomic factors including homelessness and substance abuse."
According to Century Homecare, hospital readmission figures are a closely watched measure of performance and many hospitals incur financial penalties when patients are re-admitted shortly after discharge.
"From the day we receive a patient from the hospital, our goal is to keep them safe and healthy in their own home," said Elizabeth Ciera, Assistant Administrator. "We identify what we call 'barriers to compliance' and employ a multi-disciplinary team of clinicians to address these barriers. Our nurses and therapists provide the necessary treatment while our case managers help with non-medical needs such as arranging for transportation and setting up follow-up appointments."
